General Description: 
Under the administrative direction of the Executive Director of Population Health and Care Transitions and the Sr. Behavioral Health Program Specialist, The Community Social Worker will work in collaboration and continuous partnership with the identified patient populations including but not limited to mental health, substance use and patients at risk with social determinants of health. The Community Social Worker will be responsible for managing multiple clients and their individual needs. Will collaborate with individuals at risk and their families to coordinate a plan of care to meet their social and emotional needs. 
The position shall exemplify the desired Culture of Choice® and philosophies of Parrish Healthcare. 

 Key Responsibilities: 
  • Works closely with care teams within Parrish Health Network and Community Partners to include medical schools and schools of higher learning and research to coordinate patient's plan of care. 
  • Responsible for initial patient contact, coordinating referrals, assessments and developing treatment plans for Identified patient populations. 
  • Following the guidelines of the patient centered medical home, patients identified with behavioral and social determinants of health, the community social worker will establish a personalized care plan to help address their needs. 
  • Psychosocial Assessment and Interventions: Using a team approach, the Community Social Worker will promote timely access to appropriate care, increase preventative care utilization, reduce hospital readmissions and emergency room utilization, increase comprehension and understanding of medical conditions, provide crisis intervention development and promotion of and adherence to care plans. 
  • Works with marketing and outreach programs to educate Parrish Health Network care teams and the community on resources available. 
  • Responsible for creating and maintaining relationships while working within the community and key stakeholders. 
  • Knows fire, disaster and safety procedures and regulations as pertains to the work area.
  • Performs similar or related duties as assigned.
Requirements:

Formal Education:
  • Master of Social Work (MSW) degree required from a school accredited by the Council on Social Work Education. Licensed Clinical Social Worker (LCSW preferred).  
Work Experience:
  • One to two years hospital social work experience required, or 3 years of comparable clinical experience may be considered. 
Required Licenses, Certifications, Registrations:
  • Licensed Clinical Social Worker or equivalent required.
